If you want to have an item directly in the world you'll need an entity in a world sector, too.

I've created a test mod which you can unpack to check how it's done, mainly changing guids.
The recipe is near the small camp's teleporter (no focus name, well that's another story...).

You need one bone only for this soup (to show that the modding works).
